You do not play as Emma. You play as , a disgraced paranormal investigator who, three years after the mysterious disappearance of his sister, receives a VHS tape showing Emma walking into the abandoned Hawthorne Asylum.

At the bottom right of the screen, a flickering Victorian heart beats. As you explore the asylum, encountering echoes of Emma’s final days, the heartrate increases. If it reaches maximum capacity, the "Accursed" state triggers—the walls bleed, the lights extinguish, and a silhouette known only as The Groom begins to hunt you. You cannot defeat The Groom.
